We contract with Dane County Human Services to receive referrals for children eligible for Children’s Long-Term Support (CLTS).  We work with eligible children (0-22 years old) who live in Dane County.  We help families to develop an individual support plan to assure the child can live a quality life in the community.    

We will help navigate the CLTS system by:

• Working hard to get to know the child and their family
• Completing assessments to assure eligibility in the CLTS program
• Providing benefits support as needed
• Identifying needs and developing outcomes
• Authorizing services to assure a happy and healthy life in the community
